The Community Health Outreach solution delivers a set of capabilities that help you launch a healthy communities web destination, provide an authoritative inventory of healthcare services, and engage the public in key health initiatives that increase the quality of life for all.
In this topic, you’ll learn how to use the applications in the solution by assuming various roles of individuals performing these duties.

Health resources in a community can often change. New providers become available, operating hours and contact information at certain facilities change over time. This can make it challenging for citizens in your community to know where to find accurate information. A variety of health resources can be added to your inventory and shared with the public. The Health Resource Inventory app can be used by health and human service agencies to inventory health resources that will be shared with the public.
In this workflow, you will assume the role of a staff member in a health and human services agency. You have been asked to inventory known health resources in your community.
- In a browser, sign in to your ArcGIS organization, and then browse to the Community Health Outreach solution.
- View the Health Resource Inventory app.
- On the right, click the Resource Editor to ope the widget, if necessary. Scroll through the list of available health resource types and place the location of a resource of your choice on the map.
The widget includes templates for various health resource types.
- In the widget, click the drop-down arrow or use the search field to locate the resource type.
- Click the resource symbol, and then click the resource's location on the map to place the point.
Tip:
If you already have a list of health resources in a spreadsheet, you can use the Load Health Resources tool to load the entire list of testing sites.
Note:
The Load Health Resources widget will consume credits if you use an address in your spreadsheet to create the new health resource location.
- In the Resource Editor, complete the form with details about the health resource location that you placed on the map; be as complete as possible.
Tip:
This solution leverages the Operating Hours Specification and relies on a series of data standards—for example, Mo-Fr (7:00-19:00)—to render the operating hours correctly. For more information, see Populate operating hours.
- Click Save to place the point on the map.
- Repeat these steps for each health resource that you want to add to your inventory.
Review healthy community destination
Improving public health outcomes requires a robust and transparent public education campaign that provides accurate healthcare information, progress on key healthy community initiatives, and guidance needed to access assistance. A key aspect of any healthy community program is a web destination that helps the public locate healthcare providers, review key initiatives, request assistance and volunteer to help.
In this workflow, you will assume the role of a resident or member of the public. You are interested in the healthy community program, key initiatives, health resources available in the community.
- In a browser, from the Community Health Outreach solution, view the Healthy Communities hub site.
- Review the information on the site, including the Healthy Communities program focus, frequently asked questions, upcoming events, and other information provided.
- Scroll to the Programs and Initiatives section and review the healthy community initiatives in your community.
Note:
Each initiative may link to a separate ArcGIS Hub site, depending on how you configured your site.
Take healthy community pledge
A healthy community is built on active participation and recognition of important factors that influence a persons health. Raising awareness and encouraging members of the public to live healthy lifestyles is an important aspect of a public education campaign. The Healthy Communities Pledge survey can be used to create awareness of key health issues and collect public pledges to lead healthy lives. To take the pledge, complete the following steps:
- Return to the Healthy Communities hub site, if necessary, and then scroll to the Healthy Communities Pledge section.
- Click the Take the Pledge button.
- Complete the survey, and then press Submit.
Locate health resources
Access to comprehensive health care services is important for promoting and maintaining health, preventing and managing disease, reducing unnecessary disability and premature death, and achieving health equity for all. The Health Resource Locator app can be used by the public to locate health resources. To locate health resources in the community, complete the following steps:
- Return to the Healthy Communities hub site, if necessary, and then scroll to the Find Health Services section.
- Click the Find Health Services button.
The Health Resource Locator app opens and shows all health resources from your inventory.
- Use the Locate Health Services widget to search for an address or place, or click the location on the map.
- Optionally, drag the slider to increase or decrease your search radius.
The results shown in the Locate Health Services widget highlight the various health resources near the location that you entered and within your search radius.
- In the widget, click one of the results.
Tip:
If you do not see any results, search for a new location or drag the slider to increase the search radius.
Contribute missing health resources
Developing a comprehensive inventory of health care providers is challenging. New providers will open, existing providers may close, and information you have about existing providers will continuously evolve. Engaging health care providers and the public is one way you can curate a health resource inventory and respond to changes in the community. The Health Resource Feedback survey can be used by providers to add a missing health resource location. It can also be used by the public to correct inaccurate information presented in the Health Resource Locator.
In this workflow, you will assume the role of a staff member at a local health care provider. You have noticed that your health care facility is not visible in the Health Resource Locator and have been asked to add the facility and services provided.
- Return to the Healthy Communities hub site, if necessary, and then scroll to the Find Health Services section.
- Click the let us know hyperlink.
- Complete the Health Resource Feedback survey, and then click Submit.
Tip:
The feedback will not appear in the Health Resource Locator until is has been reviewed and approved by the health and human services agency in the Health Resource Inventory app. To be notified automatically when these submissions are entered, you can set up Survey123 webhooks.
Volunteer to help
Health and Human Services agencies are always looking for volunteers that can help with activities that advance the overall health of the community. The Healthy Community Outreach Volunteers survey can be used by the public to volunteer to help with various public health outreach activities.
In this workflow, you will assume the role of an engaged resident in the community. You have noticed several healthy community issues that are of interest to you and you want to volunteer your time, experiences, and skills.
- Return to the Healthy Communities hub site, if necessary, and then scroll to the Volunteer to Help section.
- Click the Volunteer Now button.
- Complete the Healthy Community Outreach Volunteers form, and then click Submit.
Manage volunteers
As volunteers offer to help with a variety of health outreach activities, health and human services staff must contact the volunteers to ensure that each volunteer has the required skills and experiences and then align their skills and experiences with appropriate volunteer opportunities. The Community Health Volunteer Manager can be used by health and human services staff to manage volunteers.
In this workflow, you will assume the role of a staff member in a health and human services agency. You have been asked to review volunteer applications and contact each volunteer after you have completed your review.
- In a browser, sign in to your ArcGIS organization, and then browse to the Community Health Outreach solution.
- View the Community Health Volunteer Manager app.
- Expand the left panel, if necessary and click the Healthy Community Outreach Volunteers map.
- From either the table or the map, select a volunteer.
The volunteer's application opens at the bottom left.
- Review the volunteer's application form.
- Click Edit button to begin editing.
- Click the Status drop-down arrow and choose a status.
- Optionally, in the comments text box, add comments to the volunteer application form.
- Click Save.
- Click Send Volunteer Email.
Your email client opens, which allows you to format the email message and attach any additional information that you want to share with the volunteer.
Review health resource contributions
Engaging health care providers and the public is one way you can curate a health resource inventory and respond to changes in the community but it is critical to review contributions made through theHealth Resource Feedback form before making them visible in the Health Resource Locator app.
In this workflow, you’ll assume the role of a staff member in a health and human services agency. You have been asked to review health resource contributions and make the necessary updates to your inventory.
- In a browser, sign in to your ArcGIS organization, and then browse to the Community Health Outreach solution.
- View the Health Resource Inventory app.
On the map, red diamonds indicate the locations marked in feedback contributions.
- On the right, click the Resource Editor button to open the widget, if necessary.
- On the map, click a red diamond.
Tip:
Current health care resources will be symbolized with a circle.
- Review the health resource contribution.
- If applicable, use the information in the contribution to create a health resource or update an existing one.
- After you finish updating your inventory, click the red diamond again to open the feedback form, and then and change the status to Reviewed.
