The Community Health Outreach solution delivers a set of capabilities that help you develop an inventory of healthcare resources, promote key community health initiatives, and increase access to healthcare services.
In this topic, you’ll learn how to use the applications in the solution by assuming various roles of individuals performing these duties.

Health resources in a community can often change. New providers become available, or operating hours and contact information at certain facilities change over time. This can make it challenging for citizens in your community to know where to find accurate information. A variety of health resources can be added to your inventory and shared with the public. The Health Resource Inventory app can be used by health and human service agencies to inventory health resources that will be shared with the public.
In this workflow, you will assume the role of a staff member in a health and human services agency. You have been asked to inventory known health resources in your community.
- In a browser, sign in to your ArcGIS organization and browse to the Health Resource Inventory app.
- From the item page, click View.
- On the left panel, click Edit. In the Editor panel, scroll through the list of available health resource types or use the Create features search text box to locate the resource type.
- Click a health resource symbol, and then click the resource's location on the map to place the point.
- In the Create features panel, complete the form with details about the resource location and click Create.
Tip:
This solution leverages the Operating Hours Specification and relies on a series of data standards—for example, Mo-Fr (7:00-19:00)—to render the operating hours correctly. For more information, see Populate operating hours.
- To edit an existing resource location, click Select in the Editor panel, and then click the resource on the map.
- Make any necessary changes in the form, then click Update.
- Repeat steps 3 through 8 for each health resource that you want to add to your inventory.
Review healthy community destination
Improving public health outcomes requires a robust and transparent public education campaign that provides accurate healthcare information, progress on key healthy community initiatives and guidance needed to access assistance. A key aspect of any healthy community program is a web destination that helps the public locate healthcare providers and review key initiatives.
In this workflow, you will assume the role of a resident or member of the public. You are interested in the healthy community program, key initiatives, and health resources available in the community.
- In a browser, browse to the Healthy Communities site.
- Review the information on the site, including the Healthy Communities program focus, frequently asked questions, upcoming events, and other information provided.
- Scroll to the Programs and Initiatives section and review the healthy community initiatives in your community.

Each initiative may link to a separate ArcGIS Hub site, depending on how you configured your site.
Locate health resources
Access to comprehensive health care services is important for promoting and maintaining health, preventing and managing disease, reducing unnecessary disability and premature death, and achieving health equity for all. The Health Resource Locator app can be used by the public to locate health resources. To locate health resources in the community, complete the following steps:
- In a browser, browse to the Healthy Communities site.
- Scroll to the Find Health Resources section and click the Find Health Services button.
The Health Resource Locator app opens and shows all health resources from your inventory.
- In the search field, type an address and press Enter; alternatively, click a location on the map.
- Optionally, drag the slider to increase or decrease the buffer distance.
The search results display various health resources near the searched location and your buffered distance.
- In the search results, click a health resource.

If you do not see any results, search for a new location or drag the slider to increase the buffer distance.
